What the fee pays for on XRP Ledger
The XRP Ledger charges a transaction cost in drops — millionths of an XRP — that is destroyed, not paid to anyone. The base cost is tiny and rises automatically when the ledger is busy, which is what keeps spam out. The figure here is the cost the ledger will accept into the open ledger right now, read live.
There is no gas and no size pricing to think about: a payment is one transaction at one cost.
Who pays it, and what Unheld charges
The payer pays the XRP Ledger fee from their own wallet, on top of the invoice amount; it goes to the network. Unheld takes 0% of the payment and charges the merchant a monthly plan priced by the number of payments, so the fee never changes what the merchant receives.
What the receiver needs on XRP Ledger, and the first payment
The sender pays the transaction cost; the receiver pays nothing. But an XRP Ledger account must exist before it can hold anything, and a never-used address is created by the first payment to it, which must carry at least the base reserve shown above. A payment below the reserve is rejected by the ledger, and the sender keeps it.
The reserve stays locked in the account and only the amount above it can be sent on; it comes back only by deleting the account. An invoice paid to a destination tag on an account that already exists carries no reserve at all.

What does the first payment to a new XRP address need?
At least the base reserve, shown live above: the ledger creates the account from that first payment and keeps the reserve locked in it. A payment below the reserve is rejected and stays with the sender. An invoice paid to a destination tag on an existing account needs no reserve.
